An implementation toolkit can be a written step-by-step process or it can be a set of practical tools that can be used as a guide to help implementers systematically execute and evaluate new and innovative initiatives, programs, or interventions. Toolkits can be tailored to meet stakeholder needs based on the parameters of the project.

Implementation toolkits are designed to help bridge the gap between innovation and practice. The study of process of the implementation, called implementation science, helps us to understand what makes for a smoother transition from knowing about a new practice to actually implementing that practice. Implementation toolkits serve as practical instruments for change. They provide a blue print for what to do, when to do it, and how to do it. Implementation teams, stakeholders charged with implementing an initiative, program, or intervention, are vitally important in the change process because they act as drivers to navigate and direct new practice.

This document, Phases of Initiative Development and Implementation, may be helpful for use as a conceptual framework when developing a new or different way to practice. It depicts a linear progression for implementation, replication, distribution, and maintenance of the initiative, program, or intervention.
