Outcomes and Evaluation

Development of Outcome Measures

The California Department of Social Services (CDSS) and the Department of Health Care Services (DHCS) are in the process of developing outcome measures for the implementation of the Core Practice Model. A draft of the Katie A. Outcomes Matrix was developed by the Performance and Outcomes System (POS) workgroup. The matrix lists six outcome domains:

  1. access
  2. engagement
  3. services appropriate to need
  4. effectiveness
  5. linkages
  6. culturally appropriate services

New information and guidance regarding outcome measures and evaluation will be posted as they become available.

The Information Notice of March 28, 2014, provides counties with a revised progress report template for reporting progress and outcomes. Counties are required to complete the following Progress Report Enclosures by the due dates identified in the Information Notice:
